SMILE (Small Incision Lenticule Extraction) is the latest advancement in laser vision correction, designed to treat myopia (nearsightedness) and astigmatism. This minimally invasive procedure delivers results similar to LASIK, but with a gentler approach. Also known as LALEX (Laser Assisted Lenticule Extraction), SMILE has restored clear vision to more than 10 million people worldwide since its introduction in 2007.

Unlike traditional laser surgeries that reshape the surface of the eye, SMILE removes tissue just beneath the surface through a small opening. This smaller incision can lead to faster recovery times and less post-operative discomfort compared to LASIK. Am I a Candidate for SMILE?

Many people are drawn to the advantages of SMILE and choose it as their first option for laser vision correction. Similar to LASIK, SMILE is most suitable for adults with stable vision who are in good overall health. However, individuals with certain eye conditions—such as cataracts, glaucoma, or keratoconus—may not be good candidates for SMILE, though other options may be available.

SMILE can treat nearsightedness (myopia) up to -10 diopters and can also correct astigmatism. Unfortunately, SMILE is not currently available for farsightedness (hyperopia). Even if you’ve been told you’re not a candidate for LASIK, you may still be eligible for SMILE.

During your complimentary screening evaluation, you’ll undergo a comprehensive analysis in our state-of-the-art imaging suite. This detailed evaluation allows us to thoroughly assess your eyes’ unique anatomy and optical properties, helping us determine if SMILE is the right solution for you. If SMILE isn’t the best option, we’ll explore other refractive procedures such as PRK, EVO ICL, Refractive Lens Exchange (RLE), or LASIK to ensure we find the procedure that’s tailored to your vision needs.

What to Expect on the Day of Your SMILE Procedure

SMILE is a quick and effective in-office procedure. You can wear your normal clothes, but you’ll need to bring someone to drive you home afterward. The entire procedure only takes about 15 minutes for both eyes, using a single laser that completes its work in just a few seconds. Most patients choose to take an oral medication beforehand to help them relax. Your eyes will be completely numbed during the procedure, so you won’t feel anything while the laser does its job.

During the surgery, advanced laser technology creates a small, contact lens-shaped layer (called a lenticule) and a tiny tunnel in the cornea. Dr. Merkley will then remove the lenticule, which reshapes your eye and improves your vision.

Once you’re home and the numbing medication wears off, you may experience some discomfort for about two hours. During this short period, it’s common to feel some stinging or burning, along with tearing. One of the advantages of SMILE over LASIK is that the discomfort tends to be milder and lasts for a shorter time. After the procedure, you’ll need to use medicated eye drops for about a week. SMILE typically causes less dryness than LASIK, so you may not need as many lubricating drops.

With this minimally invasive surgery, there’s no flap to heal, meaning you can return to most activities within 24 to 48 hours. We’ll be with you every step of the way, ensuring a smooth experience and a fast recovery.
